网上报名 Online Registration

Instructions:

1. This form contains six (6) parts, please fill out this form before submission.
2. The required fields are marked in red asterisk ().
3. Kindly please check and make sure contact informations are correct, our staff will use these information to get in touch with you.
4. No amendment is allowed after you submit your application.
5. Before applying, please prepare the following documents:
   • ID card (front and back) (scanned copy)
   • School leaving certificate (scanned copy)
   • SPM/UEC/IGCSE results certificate (scanned copy)
   • Bank Statement (with Letterhead)
6. After submitting the documents, the system will redirect you to the FPX payment page for the registration fee. The registration will only be considered successful after the payment is completed.

3. 我们将会使用您所提供的联络方式与您联络，请确保联络资料正确无误。
4. 表格提交后将无法再修改所提交的资料。
5. 报名前，请准备以下申请文件：
   • 身份证（前页和背页）（扫描版）
   • 离校证书（扫描版）
   • SPM/UEC/IGCSE 成绩证书（扫描版）
   • 银行对账单（需含银行信头）
6. 提交资料后，系统将跳转至报名费 FPX 转账页面。只有完成转账，报名才视为成功。
